How does Lamogin 50mg Tablet work?

This medication is an angiotensin-II receptor blocker (ARB) that lowers blood pressure by blocking angiotensin, a hormone that narrows blood vessels. Preventing angiotensin's effects allows blood vessels to widen, improving blood flow and reducing blood pressure.

• Contact your doctor if your child gets her first period while on treatment with Lamogin 50.
• If you are of childbearing age, use effective contraception without interruptions during your entire treatment with Lamogin 50.
• Consult your doctor immediately if you become pregnant while taking Lamogin 50.
• After taking Lamogin 50 if you have thoughts of harming or killing yourself (suicidal behavior), stop taking Lamogin 50 and immediately contact your doctor.

Lamogin 50 is prescribed for:

• Combination therapy for partial-onset seizures, primary generalized tonic-clonic seizures, and generalized seizures associated with Lennox-Gastaut syndrome in patients aged 2 years and older.
• Monotherapy for epilepsy in individuals aged 16 and older.
• Treatment of bipolar disorder in adults.

Pharmacology

The exact mechanism of action of lamotrigine is not fully understood. It is believed to work by inhibiting voltage-sensitive sodium channels, stabilizing neuronal membranes, and modulating the release of presynaptic excitatory neurotransmitters. Lamotrigine blocks sodium currents by selectively binding to inactive sodium channels, which suppresses the release of the excitatory amino acid glutamate. This mechanism is thought to be similar in both its anticonvulsant and bipolar disorder treatments. Additionally, lamotrigine may bind to various receptors, which could explain its efficacy in treating neuropathic pain.

Dosage & Administration of Lamogin 50

Epilepsy

For patients older than 12 years:

• Weeks 1 & 2:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 25 mg daily
  • With Valproate: 25 mg every other day
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 50 mg daily
• Weeks 3 & 4:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 50 mg daily
  • With Valproate: 25 mg daily
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 100 mg daily (in 2 divided doses)
• Week 5 onward to maintenance:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: Increase by 50 mg/day every 1 to 2 weeks
  • With Valproate: Increase by 25 to 50 mg/day every 1 to 2 weeks
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): Increase by 100 mg/day every 1 to 2 weeks
• Usual maintenance dose:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 225 to 375 mg/day (in 2 divided doses)
  • With Valproate: 100 to 200 mg/day (with Valproate alone) or 100 to 400 mg/day (with Valproate and other drugs that induce glucuronidation) in 1 or 2 divided doses
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 300 to 500 mg/day (in 2 divided doses)

For patients aged 2 to 12 years:

• Weeks 1 & 2:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 0.3 mg/kg/day in 1 or 2 divided doses
  • With Valproate: 0.15 mg/kg/day in 1 or 2 divided doses
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 0.6 mg/kg/day (in 2 divided doses)
• Weeks 3 & 4:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 0.6 mg/kg/day (in 2 divided doses)
  • With Valproate: 0.3 mg/kg/day in 1 or 2 divided doses
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 1.2 mg/kg/day (in 2 divided doses)
• Week 5 onward to maintenance:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: Increase every 1 to 2 weeks by 0.6 mg/kg/day
  • With Valproate: Increase every 1 to 2 weeks by 0.3 mg/kg/day
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): Increase every 1 to 2 weeks by 1.2 mg/kg/day
• Usual maintenance dose:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 4.5 to 7.5 mg/kg/day (maximum 300 mg/day in 2 divided doses)
  • With Valproate: 1 to 3 mg/kg/day (with Valproate alone) or 1 to 5 mg/kg/day (maximum 200 mg/day in 1 or 2 divided doses)
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 5 to 15 mg/kg/day (maximum 400 mg/day in 2 divided doses)

Bipolar Disorder

For adults:

• Weeks 1 & 2:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 25 mg daily
  • With Valproate: 25 mg every other day
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 50 mg daily
• Weeks 3 & 4:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 50 mg daily
  • With Valproate: 25 mg daily
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 100 mg daily (in divided doses)
• Week 5:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 100 mg daily
  • With Valproate: 50 mg daily
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 200 mg daily (in divided doses)
• Week 6:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 200 mg daily
  • With Valproate: 100 mg daily
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): 300 mg daily (in divided doses)
• Week 7:
  • Without Carbamazepine, Phenytoin, Phenobarbital, Primidone, or Valproate: 200 mg daily
  • With Valproate: 100 mg daily
  • With Carbamazepine, Phenytoin, Phenobarbital, or Primidone (without Valproate): up to 400 mg daily (in divided doses)

Interactions

Lamotrigine interacts with oral contraceptives, Atazanavir/ritonavir, Carbamazepine, Levetiracetam, Lithium, Lopinavir/ritonavir, Olanzapine, Phenobarbital/primidone, Phenytoin, Pregabalin, and others.

Hypersensitivity

Pregnancy & Lactation

Lamotrigine is classified as Pregnancy Category C. It is excreted in human milk, and due to the potential for serious adverse reactions in nursing infants, a decision should be made to either discontinue breastfeeding or the medication, considering the drug's importance to the mother.

Precautions & Warnings

Serious skin rashes, blood dyscrasias, suicidal behavior, and aseptic meningitis can occur in both adults and children.
